Job Title: Rehab Coordinator                

Department: Home Care

Status: Full Time                

Shift: Monday through Friday Days, Some weekend and on called

Overview: Providing supervision to all Rehab services and Rehab personnel.

Principal Requirements:

  • Licensed and currently registered to practice in New York State.
  • Ability to function in the RN scope of practice.
  • BSN or Baccalaureate Degree in Health or Human Services and 2 years experience in Home Care or 4 years experience in Home Care and 6 credit hours, or the equivalent, of education/training in public health and principals of management.
  • BSN preferred.
  • Primary function to coordinate all Rehab services and supervise Home Health Aides.
  • Knowledge of OASIS, DOH regulation, federal regulations, Medicare and Medicaid, & JCAHO.
  • Home Care experience preferred.
  • Experience in Rehab preferred.

Job Title: RN - Intake Coordinator                    

Department: Home Care

Status: Full Time                  

Shift: Monday through Friday Days - Some Weekends On Call

Overview: To determine eligibility and process discharge placement for Home Care services.

Principal Requirements:

  • Must have current New York State Registered Nurse License.
  • BSN preferred.
  • Must have at least 2 years Home Care experience.
  • 1 year Home Care Intake experience preferred.
  • Must demonstrate knowledge of discharge process and utilization of Home Care services.
  • Must demonstrate knowledge of Medicare/Medicaid eligibility regulations and third party regulations.
  • Must have valid NYS Driver’s License and own a car.
  • Must have excellent communication and organizational skills.
  • Must interact successfully with Patient, Patient families, Nursing, Physicians, Social Services, outside agencies and all other staff.

Job Title: Registered Nurse

Department:Nursing - Central 3

Status: 2 Part - Time Positions (2/5)

Shift: 7am to 3pm

Overview:

Providing care within the RN scope of practice.

Principal Requirements:

Licensed and currently registered to practice as a professional nurse in New York State.

Ability to function in the RN scope of practice.

Weekend rotation required.

Ability to interact successfully with patients, families, doctors and other staff members.


Job Title: Registered Nurse                              

Department: Nursing 3 West (Psychiatry)

Status: Full Time          

Shift: Night Flex

Overview:

Providing care within the RN scope of practice.

Principal Requirements:

Licensed and currently registered to practice as a professional nurse in New York State.

Ability to function in the RN scope of practice.

Weekend rotation required.

Experience in psychiatric setting strongly preferred.

Must be able to interact with potentially violent patients.

Ability to interact successfully with patients, families, doctors and other staff members.


Job Title: RN Supervisor                        

Department: Home Care           

Status: Full- Time         

Shift: Mon-Fri. days, some weekend & on-call   

Overview:

Providing supervision to Nursing Staff

Principal Requirements:

Licensed and currently registered to practice in New York State.

Ability to function in the RN scope of practice

BSN or Baccalaureate Degree in Health or Human Services and 2 years experience in Home Care or 4 years experience in Home Care and 6 credit hours, or the equivalent, of education/training in public health and principals of management.

BSN preferred.

Primary function to supervise, instruct and guide nursing and auxiliary personnel in providing high quality nursing services.

Must be knowledgeable of federal and state regulations, JCAHO, OASIS, Medicare and Medicaid reimbursement and the Long Term Home care Program

Minimum 2 years Home care experience required.

Department: Nursing – Central 3

Status: Full Time

Shift: 7am to 3pm

Overview:

            Providing care within the RN scope of practice

Principal Requirements:

Job Title: Registered Nurse                     

Department: Nursing ICU/CCU

Status: Full Time                                   

Shift: Day Flex

Overview:

Providing care within the RN scope of practice.

Principal Requirements:

Licensed and currently registered to practice as a professional nurse in New York State.

Ability to function in the RN scope of practice.

Weekend rotation required.

Ability to interact successfully with patients, families, doctors and other staff members.

Critical care experience preferred.


Job Title: Head Nurse

Department: Nursing- 2 West

Status: Full-Time Shift, Days; rotating weekends

Overview:

Responsible for ensuring the delivery of patient care and overseeing daily functions of the unit on a daily basis.

Principal Requirements:

Licensed and currently registered to practice in New York State.

Ability to function in the RN scope of practice.

Minimum 5 years med. / surg experience preferred.

Previous management experience preferred.

BSN preferred.

Status: Full Time                                    

Shift: 3pm to 11pm

Overview:

Providing care within the RN scope of practice.
