Job Title: Social Work Assistant
Department: Social Services
Status: Full Time
Shift: Monday to Friday - 9am to 5pm
Overview: Performs variety of Social Services functions for the needs of the Komanoff Center.
Principal Requirements:

BSW or Bachelors Degree in Human Services Field.

Minimum 2 years experience in Human Services/ Social Work field.

Long Term Care preferred.

Must be able to multi task in high pressure environment.

Must demonstrate ability to use computer in Windows environment.

Must possess excellent communication, interpersonal and organizational skills.

Knowledge of MDs, Care Planning, Discharge Planning and Assessment preferred.

Familiarity with Joint Commission & DOH regulations a plus.

Must be able to interact successfully with residents, families, outside venders, regulatory agencies and other staff.

Knowledge of Medicare, Medicaid, 3rd party insured regulations and Medicare part D regulations a plus.
