Job Title : Social Worker
Department: Medical Social Service- Family Care Center
Status: Full-Time Shift: Mon.- Fri. 9am-5pm
Overview: Responsible for social work coverage for Family Care Center .
Principal Requirements:
• Must be able to read, write and speak English and Spanish fluently.
• MSW required.
• LMSW preferred.
• Must demonstrate ability to do mental health screening.
• Must demonstrate ability to do short term individual/ family counseling.
• Must demonstrate ability to perform crisis interventions.
• Must demonstrate ability to evaluate medical data, & treatment plan.
• Must demonstrate appropriate patient and family interview skills.
• Must have knowledge of local resources and referral agencies and ability to investigate and utilize community and agency resources.
• Must demonstrate ability to provide counseling related to diagnosis.
• Post graduate hospital social work experience preferred.
• Must be able to interact successfully with patients, families, physicians, staff, outside agencies, vendors and health care facilities.
