Long Beach Medical Center Home Health Care is part of an integrated multi-disciplinary system which includes The Medical Center Hospital, The Komanoff Center for Geriatric and Rehabilitaitve Medicine, Certified Home Health Agency for Short Term Care, and Long Term Home Health Care.

Certified Home Health Agency for Short Term Care

Working with the doctor, Home Care creates a special health care program designed to make each patient's recovery as comfortable and successful as possible.

This personalized program is coordinated by a Registered Nurse who is licensed by New York State and specially trained in home care.

Our team of nurses, therapists, social workers and home health aides all work together to help restore each patient to the best possible health... as they recover from surgery, heart attack, stroke, fracture or debilitating illness.

Our hospital-based Home Care has access to a full range of services and staff, 24 hours a day, every day of the week.

Who is eligible?

You don't have to be hospitalized to qualify for Home Care. With conditions such as diabetes or severe arthritis, no hospital stay is required. The doctor's recommendation brings Home Care to patients who need intermittent skilled nursing or rehabilitative care.

Insurance Coverage

Home Care coordinates private insurance, Medicare, Medicaid, and other insurance. Private payment and a sliding scale of fees can also be arranged. We work with each patient and family.

Long Term Home Health Care

Here is the alternative to nursing home placement. Through these services and community supports, many people are able to continue living at home with dignity and independence.

Commonly called "Nursing Home Without Walls", Long Term Home Health Care provides the services that meet each patient's particular needs. The professional team offers a social and medical network with backup from The Medical Center and The Komanoff Center for Geriatric and Rehabilitative Medicine.

In addition to nursing, medical, rehabilitation and personal care services, this program can provide respite care, home maintenance and housing improvement services, moving assistance, home delivered meals, social transportation and day care. Our Hospital-based Home Care has staff available 24 hours a day, every day of the week.

Who is eligible?

Nassau County residents who would otherwise require nursing home or health related facility services may apply for Long Term Home Health Care, with their doctor's approval. Planned visits by health professionals and family or friends are required by this program.

Cost of services

Long Term Home Health Care is covered by Medicaid. Private payment and sliding scale fees can be arranged.
